Dinner forms test

Zero Zero reservation policy requires that credit card information be submitted to secure a table for parties. We will ask for credit card details when calling to confirm party details. The credit card provided will not be used for payment until the evening of the event and the card must be present at time of payment. Failure or refusal to provide credit card information will result in the forfeiture of your reservation and this menu arrangement.
The Initial Charge (Guaranteed party size x package selected) plus 8.63% tax, 5% SF off set & 20% Gratuity will not be decreased for any reason. Cancellations made after 48 hours will be subject to a fee of $20/person by guaranteed party size. Please confirm party size prior to 48 hours before the event. Please note that any special requests or dietary restrictions requiring adjustments or substitutions to this menu will require approval from the Executive Chef and a minimum of four (4) days’ notice. Beverages are charged by consumption at menu price unless otherwise agreed to before the event in writing.
